Collection notice: The Department of Health (‘Department’) is collecting your information in order to process your application. Personal information collected by the Department is handled in accordance with the Information Privacy Act 2009. The personal information provided by you may be disclosed to the individuals and entities listed in your application for verification purposes. It will also be disclosed to third parties, to support the management of your application. Additionally, your personal information will be disclosed to authorised users of the Database of Research Activity. Your personal information will not be disclosed to other third parties without consent, unless the disclosure is authorised or required by or under law. If personal information is not provided, the Department will be unable to process your application.
